For the Foreign Employee:
- Passport (with copies of all pages)
- 3 recent photos (3×4 cm)
- Medical certificate (issued within the last 30 days)
- Educational certificates and/or work experience
- Employment contract or job confirmation letter
From the Employer/Company:
- Company affidavit/certificate (issued within 3 months)
- VAT registration (PP.20), company registration (PP.01), latest financial statements
- Social security employee list (Form SSO 1-10)
- Office location map & office photos
- Shareholder list (Form BorJor.5)

For MOU Migrant Workers:
- Employer’s ID card and household registration
